Getting to the Bottom of Chewing Gum Myths

It's a moment many of our patients have experienced. One second you're chewing on a piece of gum, then suddenly you forget to keep chewing and swallow the entire rubbery gob whole! It's at this point you remember your mother warning you as a child that if you swallow gum it will stake a claim and take up residency in your belly for seven years. Dr. Joel Hartjes and Dr. Jon Szewczyk and our team at hartstone dental hate to take all the fun out of the mystery, but the truth is that chewing gum, when swallowed, will enter your stomach and move through your digestive system just like any other piece of food. So, if you ever accidentally swallow a piece of gum, there is no need to worry!

That being said, it's important to know that gum does not have any dietary benefits, so while it’s not exactly harmful to swallow, you still want to avoid swallowing it. If you are an avid gum-chewer, we encourage you to chew sugarless gum, especially if you are wearing braces, because gum with sugar can lead to cavities. Sugarless gum still has the same amount of flavor, but has fewer cavity-causing ingredients. In fact, many brands contain an additive called xylitol, a natural sweetener known to fight cavity-causing bacteria. Xylitol is also known to increase salivary flow as it rinses away plaque and acid.

The fact is, when the bacterium in your mouth breaks down sugar, what’s left behind is acid. This acid eats away at the enamel coating of your teeth, causing holes that we call cavities. Cavities can lead to other long-term mouth problems if they are not treated in time, so it is best to try and avoid overexposing your teeth to too many harmful substances!

How to Subtract Those Added Sugars

Sugars are the simplest carbohydrates, and are found naturally in fruits, vegetables, grains, and dairy products. Our bodies require these simple carbs for energy. (Even better, we don’t just get energy from fruits, vegetables, grains, and dairy—we also get healthy amounts of vitamins, minerals, antioxidants, fiber, and even some protein!)

Problems arise when we get too much of this good thing. Once our bodies have all the sugar they need for immediate energy, they store extra sugars in liver, muscle, and fat cells. Over time, excess dietary sugars can affect weight and blood sugar levels, with potentially damaging results.

And, of course, sugar has an immediate effect on our oral health. The oral bacteria in plaque also need sugars for energy, which they use to make acids. An acidic environment in the mouth dissolves the minerals, which keep our tooth enamel strong. And these weak spots are vulnerable to decay. A steady diet of sugar-filled foods means that your enamel is constantly under acid attack.

So it’s really no wonder we hear a lot about avoiding “added sugars” in our diets, for both our general health and for our dental health in particular. But the concept of “added sugar” can be a bit misleading. If you’re not putting five spoons of sugar on your breakfast cereal, or pouring half a jar of honey in your tea, or using a recipe that calls for a cup of corn syrup, you might think you’re avoiding added sugars altogether. And that’s just not the case.

When dieticians talk about added sugars in our foods, they mean additional refined sugars (like white and brown sugars and corn syrup) that are used in preparing or processing foods.

Unlike unprocessed foods, which have been minimally treated to keep them as natural as possible, processed foods have been baked, or frozen, or mixed with other foods, or somehow changed from their natural state. This is where “added sugars” come in. Both home cooks and commercial processors use sugar to improve flavor, provide sweetness, and extend shelf life when preparing food.

What are some of the worst offenders? Store bought or homemade desserts like cakes and ice cream are prime candidates. Sodas? Most definitely. But it’s not just the obvious culprits. Added sugars are found in many foods we usually think of as healthy, including:

  • Smoothies
  • Fruit Drinks
  • Energy Drinks
  • Granola Bar
  • Power Bars
  • Breakfast Cereal
  • Flavored Yogurt

While eating unprocessed foods is one way to make sure you get no added sugars, you can also eliminate many grams of sugar from your daily diet by trying out low-sugar recipes and choosing processed foods with little or no added sugar.

How to avoid unexpected added sugars? Fortunately, new labelling on food packages lets us know not only how much sugar is in any product, but how much added sugar has found its way into our grocery cart. Take a moment to check out labels, and avoid sugary surprises.

Your body can get all the natural sugar and carbs it needs for energy from a healthy, well-balanced diet. Lowering your added sugar intake won’t deprive your body of necessary nutrients, but it will deprive oral bacteria of their most convenient food source and reduce the amount of acids, which threaten your enamel. In fact, subtracting extra sugars is one of the easiest and most effective ways to add to your dental health!

Charcoal Toothpaste

Despite the extraordinary claims made for charcoal toothpaste, most dentists think that the accuracy of these claims is a very gray area. So, what is the theory behind using activated charcoal in your toothpaste?

Charcoal is in its natural form is a very porous substance. When mixed with oxidizing gases or chemicals at very high heat, the inner structure of charcoal becomes even more porous. This enables the “activated” charcoal to absorb chemicals. And activated charcoal, in fact, IS used as a treatment for certain poisons. Fans of charcoal toothpaste maintain that this same porosity enables the toothpaste to collect toxins, bacteria, and debris from the surface of your teeth, leading to a healthier mouth, fresher breath, and a whiter smile.

Sounds great! Should I buy some?

Maybe not quite yet.

  • Claims that charcoal toothpastes whiten teeth more than other over the counter whiteners are difficult to prove. But even using the best charcoal product, you are getting a superficial cleaning. Because charcoal toothpaste removes stains only from the surface of the enamel, it is no match for a professional whitening.
  • It’s abrasive. Harsh pastes and brushing could potentially cause thinner enamel. Thinning enamel reveals more of the darker dentin underneath, which can actually make your smile appear yellow. Abrasive pastes can be irritating for those with sensitive or compromised gum tissue. Any toothpaste you choose should never be so abrasive as to cause damage to teeth or gums.
  • If you use only charcoal toothpaste, you might not get the amount of fluoride needed to protect your teeth. And no toothpaste can take the place of regular brushing, flossing, and checkups at our Middleton, WI office.
  • If you’ve seen the photos posted of charcoal enthusiasts with sooty smiles and teeth, you know brushing with charcoal toothpaste can be a messy process. You might need to take extra care to clean your mouth, teeth, and tongue after using. And your sink.

Fluoride Use in Adolescents

Fluoride is a mineral that plays an essential role in oral health. In fact, the significant reduction in American tooth decay in recent decades can be attributed to a greater availability of fluoride in public water supplies, toothpaste, and other resources. When it comes in contact with the teeth, fluoride helps protect the enamel from acid and plaque bacteria. In some cases, it can even reverse tooth decay in its earliest stages.

Despite the benefits of fluoride, tooth decay is still common, especially among teenagers. The Centers for Disease Control reports that cavities can be found in more than half of young teens and two-thirds of older teens over age 16. Many of those teens are deficient in fluoride, either due to a lack of public water fluoridation or the use of bottled water. So how can parents ensure their teens are getting the fluoride they need to facilitate strong, healthy teeth?

Monitor Fluoride Exposure

Dr. Joel Hartjes and Dr. Jon Szewczyk and our team at hartstone dental recommend you start by measuring your teen’s fluoride exposure. Make sure you purchase fluoridated toothpaste for your household, and find out if your tap water is fluoridated. If your teen primarily consumes bottled water, examine the bottle to determine whether fluoride has been added. The majority of bottled waters are not supplemented with fluoride, but those that are will be clearly labeled.

Fluoride Supplementation

Dr. Joel Hartjes and Dr. Jon Szewczyk may recommend topical fluoride treatments at routine dental exams. These treatments are painless for your teen and may help establish stronger enamel that is more resistant to plaque and tooth decay. If you have a public water supply that is non-fluoridated, we may recommend fluoride supplementation between visits. These can be administered as drops, tablets, or vitamins.

Keep in mind that fluoride is most important for children and teens under the age of 16. Be proactive about your teen’s oral health by speaking with us about your family’s fluoride needs at your next dental visit.
